Victorian Children’s Tool for Observation and Response (ViCTOR) is a suite of standardised charts and educational resources for use in Victorian health services.

ViCTOR (paediatric wards and emergency departments)

These observation charts incorporate age-related vital signs for children across five age groups and are intended for use in paediatric wards and emergency departments.  The charts should be used in conjunction with the health services specific escalation of care procedures.

Newborn Charts

There are two types of newborn charts: one intended for the newly delivered baby in the birth suite and postnatal setting and the other for use in the special care nursery.

Urgent Care Charts

These charts have been designed for use in Urgent Care and small rural health settings. The original ViCTOR charts have been adapted and include additional sections for initial assessment and triage information, presentation/admission check list and transfer/discharge check list and a paediatric medication chart.

Fluid Management Charts

There are two types of fluid management charts: one for use in the paediatric ward and emergency department the other for use in the special care nursery.

ViCTOR Neurological Observation Chart

This neurological assessment chart is designed to be used with the ViCTOR charts and incorporates  the  paediatric coma score for three age groups (Birth to 1 year, 1- 5years and older than 5 years).

VICTOR Medical Emergency Response Metrics

This form along with the “General Instructions” guides data collection for use with inpatient paediatric patients who have a medical emergency response (e.g. MET call or Code Blue). These metrics have been developed to assist organisations in their local review of deteriorating patients.
